Ano ang Makatizen Card?
Ang Makatizen Card ay isang government issued identification card na may mga sumusunod na gamit:
➢ Kagaya ng inyong blu, yellow at white card, maaari kayong makakuha ng inyong mga benepisyo at serbisyo mula sa Makati City Government gamit ang Makatizen Card;
➢ Ito ay nagsisilbing “Makatizen Account (mobile wallet)” na kung saan maaari ninyong matanggap ang inyong mga cash benefits mula sa Makati City Government o kaya naman ay lagyan ng pera o tinatawag na “cash-in” upang magamit sa iba pang serbisyo ng card;
➢ Maaari kang makakuha ng mga loyalty at reward points sa tuwing ginagamit mo ang iyong Makatizen Card. Ang mga points na ito ay magagamit mo rin upang magamit sa iba pang serbisyo ng card.
Ang inyong Makatizen Card ay handong ng Makati City Government, ka-partner ang Globe at GCash, at iBayad. Ibig sabihin nito, ang isang tao na may Makatizen Card ay maaaring gamitin ang mga services ng GCash at iBayad gamit ang nasabing card.

What if one of the requirements is lacking or photocopies only and no original copies
✓ Applicant must bring complete requirements both original and photocopies. ✓ Incomplete requirements will not be entertained.

Can we apply anytime
Can visit the Makatizen Card Online Application Website anytime for the pre-registration. Once applicant is done with the pre-registration, he/she can visit the Makatizen Hub at 4/f SM Makati from 10:00 AM to 5:00 PM.
1. Applicant can visit COMELEC, Makati office to request for their COMELEC Certification

Is US Passport allowed as supporting document
US Passport is not allowed. If applicant is a dual citizen, applicant must present his/her Philippine Passport.

What If I have no original birth certificate
1. Applicant to get a copy of Birth Certificate from NSO/PSA. 2. If born in Makati, applicant can get a copy from City Civil Registry Office (CCRO) - Makati located at the 3rd floor of Makati City Hall Bldg. I

What if I have no record in NSO or in any local civil registry office
1. Applicant must present the original copy of no record from NSO/PSA. 2. Applicant must proceed to City Civil Registry Office (CCRO) to apply for late registration. The CCRO will issue endorsement letter to PSA/NSO for late registration. 3. Can present Passport, if there are any.

What if I have birth certificate issued by other local civil registry of other LGUs
1. If not from CCRO - Makati, applicant have to get a copy from NSO/PSA.
Note: Only Birth/Marriage Certificate issued by the Local Civil Registry Office - Makati only is accepted.

What if I have birth certificate issued by other local civil registry of other LGUs but there is no record from NSO/PSA
1. Applicant must present the original copy of no record from NSO/PSA. 2. Applicant must proceed to City Civil Registry Office (CCRO) to apply for late registration. The CCRO will issue endorsement letter to PSA/NSO for late registration.
For Senior Citizen only 1. Applicant must submit any supporting documents like baptismal, school/medical records, SSS, GSIS and other document showing full name and birthday. 2. Submitted supporting documents will be subjected for verification.

If there is a discrepancy in any document presented; like 1. in birth certificate, my name is JUAN DELA CRUZ, but in COMELEC Certificate or Presented ID, my name is JUAN CRUZ.
Applicant can choose whether to have his data captured or not, pending approval of their application. Their application will be endorsed to the Office of the Mayor for guidance and approval. Data should be consistent in all the documents/requirements.

Transferred to a new barangay and my document shows my old address. Where can I get my barangay certificate?
Applicant should get a new Barangay Certificate from the barangay where the present/new address is located/covered.
